Zyteq respects your privacy. The following are our policies as they relate to the 11 principles of the Privacy Act. This business is in fact not required to comply with the Privacy Act, however we do adhere to these principles as a courtesy.
- Manner and purpose of collection of personal information | We collect personal details for the purposes of administering our loan equipment and repair services and to provide sales. In addition personal details are collected for our mailing list. If you receive a newsletter then we have your name and address, and possibly phone number, fax number and email address. These details are collected directly from you when you telephone the Zyteq office, and we ask you directly for your details. Sometimes a therapist may provide details of a client (third party) who is to receive loan equipment, presumably with their consent. If equipment arrives here for repair we may gather personal details from the documentation that arrives with the equipment, for example if the equipment user and equipment owner are different. The information is only collected for the purpose of returning the equipment safely to the correct person, for invoicing purposes and to have sufficient contact details to process the repair or loan. The details collected include: name, facility, address, phone numbers, fax numbers, email address, serial number of equipment. Sometimes names of alternative contact people may be recorded, and their relationship to the main contact person noted. Disability or diagnosis may be recorded, in addition to age of client, and specific equipment used which may also be relevant, e.g. type of switch.
- Storage and security of personal information | Records are stored electronically on the office computers. At times details are printed out containing name, facility, address and phone numbers, for loan or repair documentation. You will always receive a copy of this. Paper files are kept for repairs. Loan forms are only kept until the end of the current calendar year. They are then destroyed. Information is not divulged to any other parties, without your prior consent. We do not sell or distribute our mailing list outside this office in any way. Records are not automatically deleted from our database unless the individual requests this, or the individual passes away and we are notified or have post returned marked in this manner.
- Access to records containing personal information | Anyone on the database is welcome to the information about themselves. Proof of identity will be required prior to information being released.
- Alterations to records containing personal information | We are always willing to update and amend records and welcome your assistance in updating your personal details.
